School Program Booking Form

To book a program, please download our new PDF booking form and complete it, then send it back to us either by fax or by email. (This form, if opened by Acrobat Reader v.6 or higher, can be easily filled in by clicking on the relevant areas and typing in the information.)

Instructions

1) Click on link to open form

2) Click on each of the text areas and type in your contact information, your preferred dates and times, etc.

then

3a) Once completed, click on the "Print Form" button to automatically send the completed for to your printer; you can then fax it to the Museum at 604-665-3585;

or,

3b) Once completed, click on the "Submit By Email" button to attach the completed form to an email, which you can send back to the Museum.
